Provides medical social services to individuals in their place of residence in collaboration with other disciplines and external agencies as related to assigned patient.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
- Makes home visits to assess patient/family/significant other’s needs for provision of medical social work.
- Maintains open communication with members of the health care team.
- Assists patient/family/significant other to understand reaction to illness and cope with associated feeling and responses.
- Provides age-appropriate education in collaboration with patient/family/significant other and members of the health care team as relevant to the psycho-social needs of the patient.
- Assists patient/family/significant other to access resources related to financial need for their medical care.
- Utilizes appropriate community resources.
- Participates in case conferences with supervisor as well as other health care members as needed identifying social problems and their inter-relatedness to the medical situation.
- Documents the delivery of social work services to comply with regulatory requirements, agency policy, procedures and standards of practice.
- Reviews and co-signs all Bachelor of Social Worker (BSW) documentation and offers appropriate recommendations as needed.
- Conducts assessment visit prior to employee’s completion of introductory period, then yearly.
- Displays time management and organizational skills.
SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:
- Accountable for utilization and practice of systems thinking, teamwork and customer service.
- Follows all agency policies and procedures.
- Participate in mandatory in-services and educational activities as required.
- Other duties as assigned.
MINIMUM REQUIRED QUALIFICATIONS:
- Master’s degree (MSW) from a school of social work accredited by the Council of Social Work Education
- One year of social work experience in a health care setting.
- CPR certification
- Use of personal vehicle, valid PA driver’s license and auto insurance.
- Two (2) years of social worker experience in the healthcare field.
- One year of Home health experience preferred.
The position requires the ability to attend to more than one aspect of a situation simultaneously. It is highly likely that multiple task demands are going to be required of the individual at the same time.
New Learning and Memory:
The position requires that an individual be able to learn new tasks quickly and effectively. Job requirements change frequently. The ability to understand and carry out detailed, involved instructions is mandatory.

The position required much autonomy of thought and problem solving. The individual must be able to apply principles of logical or scientific thinking to define problems. The individual must be able to think abstractly, which is manifested in the ability to form concepts, use categories, generalize from single instances, apply procedural rules and general principles, and be aware of subtle or intrinsic aspects of a problem. The development of hypothesis and potential solutions to problems involves careful interpretation, analysis and diagnosis. The individual must be able to collect data, establish facts, and draw valid conclusions. The individual must be able to think creatively with a degree of inventiveness, experimentation and intuition. They must be able to deal with a variety of concrete and abstract variables.
