1.0 FTE, day shift, no call, no weekends

Job Description

HOURS: full time, day shift between 7pm-4pm or 8am-5pm, no weekends, no call

This role will have the opportunity to lead a US News and World report top 50 US Orthopedic program at Penn Medicine Lancaster General Health (LGH). LGH is currently ranked #40 in the US for orthopedics by US News and World Report and has received JC certification for Hip and knee replacement as well as hip fracture. LGH also achieved Premier Level certification in hip fracture management by the International Geriatric Fracture Society, one of the first hospitals in the United States to achieve this certification.   

  • Current state board licensure as a Registered Professional Nurse, or Physical Therapy
  • Bachelor’s degree in nursing, required degree in allied health profession, healthcare management or related field
  • Three (3) or more years of experience as a clinician

POSITION SUMMARY: Leads the organization’s efforts around service line program development, generally in one to two disease specific focus areas. Collaborates in the management and navigation of a complex disease specific patient group(s) over an entire episode of illness, performing clinical and administrative functions to support the coordination of inpatient and/or outpatient care needs. The supervisor is responsible for assisting in the development, implementation, and on-going evaluation of the program services with the goal of maintaining and enhancing the quality of care delivered to patients while ensuring applicable regulations are met. 

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Manages the care of complex patients, evaluates patient care outcomes, plans corrective action for identified variances, and utilizes business intelligence tools to collect and aggregate patient data
  • Collaborates with nursing staff, physicians, and other members of the health care team to achieve desired patient outcomes while managing resources in an effective and efficient manner
  • Demonstrates proficiency in applying clinical theories, evidence based nursing knowledge, problem solving and therapeutic techniques to perform professional duties safely, legally, ethically, efficiently, and effectively.
  • Knowledge of pathophysiology of specific disease processes and understands treatment modalities and rationale such as: surgery, therapy, symptom management, diet and self-care strategies, survivorship, surveillance and advanced care planning
  • Assist in identifying and managing physical, mental, psychological and/or spiritual well-being while working with the patient and caregivers when developing a pathway or treatment plan and contribute to its implementation and evaluation. Develops a therapeutic relationship with patients and caregivers and maximizes participation and control over the disease management process.
  • Provides education to patient/caregiver on items such as: disease/symptom management, medication purpose and side effects, diet, rehabilitation, and when to notify the healthcare provider. Provides information on community resources and agencies.
  • Leads and/or actively participates in clinical effectiveness teams
  • Participates in related national and local professional organizations, reviews literature and assess trends in patient care needs and develops services and resources to meet those needs.
  • Assessment of patients ability to navigate through the healthcare system and identify potential barriers. Guide patient through the healthcare system to ensure:
    • Appropriate referrals are made to the right provider at the right time
    • Strong communication between specialty physician practices within the organization – building trusting relationships with physicians and staff
  • Identify and implement metrics and methodologies for continuous quality improvement within the service line and support and/or lead disease specific performance improvement initiaitives
  • Actively engage and participate in community outreach, education and supportive services programs, including appropriate screening programs or educational and health awareness promotion activities. Serves as a resource and provides relevant education to Lancaster General and affiliate healthcare workforce.
  • Supervises clinical and non-clinical staff in area of specialty. Provides oversight and accountability for any applicable databases, registries and registry staff, ensuring accurate and timely data abstract and data submissions

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Participates in related committees, multi-disciplinary boards and/or service line activities as appropriate
  • Provides clinical rounds to ensure appropriate processes and pathways are being followed
  • Communicates when needed with insurance case managers ad representatives to provide information regarding patient outcomes.
  • Participate in relevant research and grant activities
  • Other duties as assigned


  • Current state board licensure as a Registered Professional Nurse, or Physical Therapy
  • Bachelor’s degree in nursing, required degree in allied health profession, healthcare management or related field
  • Specialty certification within 3 years of hire in related area such as:
    • Dementia Practitioner
    • Clinical Nurse Specialist
    • Case Management
    • Orthopedics
    • Neurology 
    • Informatics 
    • Pain Management
    • Stroke
  • Three (3) or more years of experience as a clinician


  • Five (5) or more years of experience as a nurse or clinician in related specialty
  • Masters of Science Degree Nursing (MSN) or Masters/Doctorate in Physical Therapy
  • Experience in care coordination and patient education
  • Case Management experience (including inpatient and outpatient)
