Job Description
Population Health Fellowship:
Penn Medicine Lancaster General Health is seeking a BE/BC physician to train within a full or part time Population Health Fellowship. The combination of high-quality health care services and an institution-wide dedication to community care makes Lancaster General Health the perfect location to develop skills in population health.
Interdisciplinary education, including both team development and modeling, is an integral part of our Population Health Fellowship. Time in the fellowship is split between clinical work in Care Connections and work within our Accountable Care Organization, Lancaster General Community Care Collaborative.
The fellowship training allows the fellow to earn a Master’s of Population Health or a certificate program in population health from a partner institution such as the University of Pennsylvania, Temple University or Jefferson University. Along with your education, you will gain experience teaching in the Lancaster General Health Family Medicine Residency.
The fellow’s clinical time will be focused on high-risk medical community care through our Care Connections team. Care Connections is an innovative, intensive, interdisciplinary and transitional primary care medical home that Lancaster General Health has developed to showcase how we are transforming healthcare.
The aim of Care Connections is to better serve patients with three or more medical problems plus behavioral health issues, who have been admitted to the hospital multiple times in the past year. The program provides quality care with a customized experience, while lowering overall healthcare costs.
Support for the medical team is provided by patient care navigators, social workers, a county social services liaison, care management nurses, a behavioral health psychologist and a clinical pharmacist. Care Connections receives support from the County of Lancaster, the Commonwealth of Pennsylvania and multiple community stakeholders. Most importantly, Care Connections has been built with the assistance of our patients to ensure that their care will always be our first priority.
About the Fellowship:
- A flexible one to two-year program

Goals and Objectives:
- Training effective leaders in population health and accountable care
- Fostering and expanding clinical, analytical and research skills
- Increasing physician exposure to and challenging creative thinking on current issues in population health
- Developing new physician leadership for LG Health through active participation in Care Connections activities, coursework and attending/convening work groups
- Enhancing the ethical capacity needed for management of population health and extensivist care, in health systems
