Job Title: Pharmacist Per Diem
This position is located in Lancaster, PA
Casual 0.1 looking for a candidate available to cover 10-20 hours per week Monday through Friday and at least one Sunday a month . Work as Ante Room Pharmacist to check and verify compound IV medications. Will also be trained in clinical work. Varied 4-8 hour shifts between 6:00 am-6:00pm Monday-Friday and 8:30 am to 5:00 pm Sundays( at least one Sunday a month). Located at Horizon Healthcare Services

SUMMARY: Lancaster General Health/Penn Medicine is currently seeking a Pharmacist for Horizon Home Care Services. This position will work Per-Diem as an Ante Room Pharmacist to Check and Verify Compound IV meds. The pharmacist assists healthcare providers in utilizing drug therapy safely and effectively through medication prescription review and fulfillment, patient monitoring and patient education. Supervises pharmacy interns, students and technicians in their tasks, and verifies all work completed by those personnel. Patient care includes care to patients in all age groups.
Accurately compounds, labels and dispenses medications per physician orders utilizing appropriate techniques and completing proper documentation (including lot numbers and expiration dating information) within established timeframes.
Interprets and evaluates physician medication and IV admixture orders, obtaining clarification as deemed necessary, and carefully reviews the prescription, retrospectively and prospectively, to prevent adverse reactions, allergic reactions, drug interactions and improper dosing. Consistently recognizes inappropriate physician orders and takes action to resolve problems without resulting delays in patient treatment. Documents findings and interventions appropriately.
Controls the disposition of narcotics and other scheduled drugs as required by State and Federal regulations. Assists with inventory control for pharmaceutical products and accessories.
Responds to drug information requests as required, effectively utilizing library and reference sources. Accurately & effectively counsels patients on the appropriate use of medications & responds to drug information requests in a helpful & professional manner. Provides patients with the basic information necessary to safely administer medications.
Thoroughly reviews all products made by a pharmacy technician for accuracy and correctness & resolves problems prior to products being dispensed.
Monitors Clinical Therapy:
Consistently follows established policies regarding patient assessments and re-assessments.
Demonstrates a thorough knowledge of the care planning process. Initiates appropriate care plans at the start of therapy. Through communication with patients, nursing staff and physicians, monitors medication effectiveness and makes recommendations for therapy changes as appropriate, and completes and updates the Pharmaceutical Care Plan as required.
Reviews labwork in a timely manner and responds to abnormal lab values by making appropriate recommendations to the prescribing physician.
Actively participates in the CQI program, making recommendations for process improvement and identifying processes requiring intensive examination and study.
MINIMUM REQUIRED QUALIFICATIONS:
B.S. degree in Pharmacy, or PharmD.
Current, valid Pennsylvania Pharmacist license in the Commonwealth of Pennsylvania (or eligible to reciprocate)
Familiarity with computer-based operating and reporting systems and knowledge of network computer usage (or ability and willingness to be trained.)
Compliance with federal, state and other funding agents: A healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e., not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.
Minimum of two (2) years’ experience as a pharmacist preferred.
Hospital or home-infusion pharmacy experience preferred.
This position requires a Professional License/Certification.
