Job Description

HOURS: Part-Time (20hrs/wk); 8:30AM-5:00PM; schedule may require some flexibility determined by meetings and workload.

POSITION SUMMARY: This position will operate at systems level working with cross functional teams focusing on advancing and standardizing patient education to impact health outcomes, improve patient engagement and hospital re-admissions.  

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Standardize patient education across the continuum.
  • Evaluate patient documents for readability.
  • Maintain a detailed understanding of patient education elements and functionality in Epic
  • Optimize and maximize Healthwise contract and tool implementation.
  • Coordinate staff education and competencies for teach-back, motivational interviewing and goal setting.
  • Train staff who will educate patients.
  • Ensure that the organization's quality and regulatory requirements are met.
  • Develop metrics to enable tracking and ability to determine the success of the training program (i.e., how well are patients learning?).
  • Member of system-wide patient education committee


  • Bachelors degree in Public Health, Education/Health Education or Registered Nurse is required.
  • Three (3) or more years of experience in hospital or related educational role is required.
  • Strong program development skills is required.
  • Strong understanding of plain language and health education principles is required.


  • Master degree in Public Health, Education or Health related field or current licensure as a Registered Professional Nurse, issued by the PA State Board of Nursing is preferred.
  • Experience with motivating and engaging diverse partners.
  • Two (2) years of leadership experience.
