Penn Medicine Lancaster General Health is seeking a full-time Nurse Navigator for our growing Stroke Program.  The Nurse Navigator will work collaboratively with our expanding Neurology team to provide education to both patient and their support system along with clinical navigation supporting the coordination of inpatient and/or outpatient care needs for our stroke patients.  This position will focus on creating a seamless transition for the stroke patient during their recovery phase from hospital to next level of care and ultimately assisting them in achieving the goal of home.   As a forward-thinking Neurology practice, we are a Joint Commission-Certified Primary Stroke Center and one of the busiest stroke centers within the Penn Medicine Network. Our program at Penn Medicine Lancaster General Health is the first and only hospital within Lancaster County to perform mechanical thrombectomy.  Our program is nationally recognized for its stroke care by the American Heart Association/American Stroke Association’s Get With The Guidelines®-Stroke Gold Plus and Target: Stroke Honor Roll Elite Quality Achievement AwardThe award, according to the American Heart Association/American Stroke Association, "recognizes the hospital’s commitment to ensuring stroke patients receive the most appropriate treatment according to nationally recognized, research-based guidelines based on the latest scientific evidence".

HOURS:  Full-time, Primarily day shift. Monday - Friday, occasional weekend


  • Current licensure as a Registered Professional Nurse, issued by the PA State Board of Nursing.
  • Bachelor’s degree in nursing, healthcare management or related field.
  • Certification in related specialty (Stroke Certified RN - SCRN) required within 3 years of hire
  • Three (3) or more years of experience as a clinical nurse in related specialty.


  • Five (5) or more years of experience as a clinical nurse in related specialty.
  • Masters of Science Degree Nursing (MSN).
  • Experience in care coordination and patient education.
