Summary
POSITION SUMMARY:
Manages Community Health and Wellness staff. Plans, implements and evaluates community health improvement initiatives as stated in the Community Health Improvement Plan (CHIP). Manages current grants and proactively identifies new grants.
HOURS: Monday-Friday day time hours with occasional evening/weekends based on community events and meetings.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Develops, plans, implements and evaluates community health improvement initiatives.
Coordinates the development of patient care standards, protocols, and guidelines. Ensures clinical staff is knowledgeable about patient care standards. Acts as a change agent by influencing attitudes, modifying behavior, and introducing new approaches to community health services.
Monitors and maintains staff patterns in clinics
Counsels and coaches employees and, as necessary, takes appropriate disciplinary action.
Guides and evaluates the performance and development of each staff member with the goal of further developing/recognizing strengths and suggesting additional training on areas of improvement.
Interviews and hires qualified personnel with staff input.
Prepares and controls budgets.
Assess and analyze community health data & identify gaps and needs in services or initiatives.
Serves as a role model for wellness.
Tracks reports, data and statistical as needed based on program/interventions.
Collaborates with internal and external customers to achieve department goals.
Develops & maintains trusting relationships with key external community leaders.
In collaboration with grants department, identifies, writes and monitors grants to meet deliverables and advance initiatives.

JOB REQUIREMENTS
MINIMUM REQUIRED QUALIFICATIONS:
Bachelor's degree in Public Health, Health Education, or related field.
Three (3) to five (5) years of Public Health or related experience.
One to two (1-2) years of managerial or supervisory experience.
One to two (1-2) years of program development experience.
PREFERRED QUALIFICATIONS:
Master’s degree in Public Health, Health Education, or related field.
