Lead Family Advocate, (Full Time)


Job Description

HOURS: Full Time (1.0 FTE; 40hrs/wk) occasional evenings/weekends.


POSITION SUMMARY: The Lead Family Advocate will provide case management support to children and their caregivers that reside in a property that has been identified with lead hazards and/or have been affected by lead poisoning.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Communicate with professionals and other community agencies such as to ensure childhood lead poisoning awareness
  • Connect child to appropriate supportive services
  • Connect caregiver with the lead program
  • Coordinate relocation process for the caregiver
  • Record and track needs and progress of child's lead level
  • Develops and maintains relationships with medical staff to ensure the overall well-being of a child.


  • Bachelor's degree
  • Two years of social worker experience in the Community Health Field
  • Excellent verbal and written communication skills.
  • Proven informal leadership skills.
  • Ability to work independently, setting priorities to coordinate care plans efficiently.
  • Ability to work effectively in a fast-paced team environment.
  • Highly organized and detail-oriented with the ability to perform multiple tasks simultaneously.
  • Effective behavioral and educational strategies, including, but not limited to : Motivational interviewing, teach-back method, and self-management support.


  • Masters Level Social Work credential
  • Family Advocate experience
  • Bi-lingual (English and Spanish)
