Summary

POSITION SUMMARY:

Assist with intake processing by fielding phone calls, contacting referrals for information, data-entering information, scheduling nursing visits and following-up with patients.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Prioritizes work according to referral requirements and deadlines. 
  • Accurately and in a timely fashion processes new patient referrals, enters data into the electronic record and communicates with HHS staff, patient start time, along with specific needs.
  • Enters new physician information, diagnosis codes into the electronic record and procures HHS nursing services or ancillary providers to start patient care.
  • Pulls and assembles standard packet documents to include referral documents, and teaching sheets (if applicable).  Ensures elements on intake checklist are completed accurately and within identified timeframes for each patient. Shares concerns with supervisor.
  • Understands the content/usage of all reports used to facilitate workflow and uses good judgment & initiative when faced with problems, issues or assignments for which clear instruction may not be available.
  • Identifies and assists in developing quality indicators and data retrieval tools.
  • Accurately tracks pending referrals by maintaining the potential new start list in accordance with established procedures.
  • Is proactive in problem-solving incomplete information to eliminate errors later in the process.
  • Consistently communicates appropriate information in a clear, concise and timely manner with   others. Shares concerns and makes suggestions for improvement.
  • Participates in Performance Improvement activities by assisting with monitoring and data retrieval as required.
  • Develops effective working relationships with referral sources, co-workers and other department staff.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Actively participates in staff meetings, recommends improvements in processes based on PI monitoring activities.
  • Demonstrates a high degree of organizational skills by keeping a neat work area & maintaining organized files and records.
  • Displays flexibility, adaptability and a positive attitude by willingly assisting and covering the assignments of other staff in their absence or unavailability.
  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • High school diploma or equivalent.
  • Two (2) years’ general office experience or equivalent training. 
  • Minimum two (2) years’ data entry/computer experience. 

PREFERRED QUALIFICATIONS:

Medical office, medical records, or pharmacy technician experience & familiarity with medical/pharmaceutical terminology preferred.
