Summary

POSITION SUMMARY: Develops initial home care service plan with patient and physician after referral to Affilia.  Coordinates initial delivery of HME, supplies, services.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Upon referral, meets with patient/family to educate about home care services and to assess home care needs, including DME, supplies, medications and, skilled services.
  • Collaborates with physician/discharge planner to obtain physicians’ order for home care services and needed supplies and equipment.
  • Completes the admission worksheet.
  • Discusses funding sources with patient/family as appropriate.
  • Collaborates with Home Care Resource staff/appropriate Alliance entity to facilitate the referral process.
  •  Functions as the Affilia liaison by developing professional relationships with referral sources.
  • Serves as agency representative in communicating information related to agency services, policies, billing, etc. to patients, families, other agencies, hospitals, physicians and members of the community.
  • Educates hospital staff of home care services/options through formal and informal encounters.
  • Identifies trends in hospital-based referral patterns and participates in activities to accommodate such changes.
  • Notifies referral source when patient does not meet admission criteria for home care and discusses alternatives.
  • Maintains records and statistics of referrals as requested.
  • Accountable for utilization and practice of systems thinking, team work and customer service.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • Current Pennsylvania LPN license.
  • Minimum of one year acute care experience.
  • Two years of home care experience required.

Mathematics: The following levels of ability are necessary to understand mathematical concepts and apply them to problem-solving situations.

  • Ability to add and subtract two-digit numbers.
  • Ability to multiply and divide.
  • Ability to perform the four basic arithmetic operations with coins as part of a dollar.
  • Ability to add, subtract, multiply, and divide all units of measure.
  • Ability to perform the four operations with like or common decimal fractions.

PHYSICAL REQUIREMENTS: Degree of physical exertion is light, exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to move objects.

BODY POSITION AND MOVEMENT: The following positions or movements are typical of the job:

  • SITTING is done frequently.
  • WALKING is done frequently.
  • STANDING is done frequently.
  • CLIMBING is done frequently.
  • REACHING (Extending out and retrieving objects outside immediate range) is done frequently.
