Family Advocate-Nurse Family Partnership (Social Work) (Full Time, Day Shift)
LOCATION: Commercial Avenue, Lancaster, PA
HOURS: Full time, 40 hours a week or 1.0 FTE. First Shift, Monday through Friday. No holidays or Weekends.
ABOUT: Spanish speaking preferred. The Nurse Family Partnership Family will see families in their homes and community and work closely with the NFP Nurse Home visitor to meet the needs of the families.
The Family Advocate with Women & Children’s Community Health will be part of a team to provide trauma informed case management, education, and support to families enrolled in the Healthy Beginnings/STEPS or Nurse Family Partnership programs.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Meets patients in a variety of settings to conduct an assessment, including helping patients to set goals.
Makes home visits to fully understand the environment and collaborate with the multidisciplinary team in the assessment of needs.
Assists patient/family to access resources related to insurance/financial/basic household needs. Assist in completing applications for assistance when needed.
Delivers needed items from community resources to the client’s home, including but not exclusively, car seats, pack in plays, food, infant care items.
Makes follow-up calls and home visits to patients.
Helps patients build their support system.
Assists in coordinating/teaching parent education groups.
Empowers and motivates patients to meet their goals and promote self-sufficiency.
Networks and maintains relationships with community resources.
Maintains open communication with members of the health care team to meet patient’s needs and goals.
Documents the delivery of services to comply with regulatory requirements, agency policy, procedures and standards of practice.
Displays time management and organizational skills.
Attends and participates in staff, committee, department, and other administrative meetings.
MINIMUM REQUIRED QUALIFICATIONS:
Two (2) years of experience in the Social Services Field.
Demonstrated comprehensive knowledge of social and community resources.
Excellent verbal and written communication skills.
Ability to demonstrate good judgment, independent thinking skills and problem solve.
Basic computer skills.
Valid Driver’s license, with a reliable vehicle, and the ability to access work assignments within our defined geographic area.
Ability to work independently in the community.
Highly organized and detail-oriented with the ability to perform multiple tasks simultaneously.
Experience in maternal child health or early childhood development.
Bilingual (English and Spanish).
Effective behavioral and educational strategies, including, but not limited to: Motivational interviewing and teach-back method.
