Coordinates patient visits and nurse assignments to ensure optimal consistency of clinicians.
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
- Create and manage the daily clinical schedules communicating as needed with staff and patients.
- Apply critical thinking by working closely with the Case Managers and Clinical Managers to ensure compliance with the case management model.
- Responsible for calculating and determining daily and weekly admission capacity.
- Review clinical referrals and assign start of care date by utilizing defined parameters. Consult with Clinical Managers as outlined in parameters.
- Work closely with the Home Care Coordinators to reassign start of care dates as availability is created on the nursing schedule.
- Create nurse staffing schedule based upon staff availability and approved time off as provided by Clinical Manager.
- Complete telemedicine monitor assignments under the direction of the telehealth department to maximize staff efficiency and minimize overtime.
- Responsible to review and follow-up on assigned weekly reports to assure documents and processes are completed as needed. Make corrections as assigned or provides feedback to staff/Clinical Managers regarding corrections to be made and/or trends noted to facilitate performance improvement.
- Respond to incoming calls from patients, caregivers, and employees to facilitate and coordinate patient care.
- Document calls promptly in accordance with agency requirements.
- Consistently demonstrate proper telephone technique and etiquette.

MINIMUM REQUIRED QUALIFICATIONS:
- High school graduate or equivalent.
- Basic typing and data entry skills.
- Ability to communicate and work effectively with internal and external customers.
- Knowledge of office equipment, copy machines, typewriter, computer, and word processor.
- Good organizational skills.
- Ability to meet the requirements of the job as described in the job analysis including, but not limited to: sitting, standing, walking, lifting, bending, and use of telephone, computer and fax.
- Skilled at problem solving, issue resolution.
- Strong time management, computer, multi-tasking, interpersonal skills.

Ability to perform the four operations with like or common decimal fractions.
EQUIPMENT USAGE REQUIREMENTS: MACHINES, EQUIPMENT, TOOLS, SOFTWARE: To perform the essential duties of the position, use of the following machines, equipment, tools, and software is customarily required:

PHYSICAL REQUIREMENTS: Degree of physical exertion is light, exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to move objects.
