POSITION SUMMARY: Assists Clinical Supervisors by processing incoming calls from patients/caregivers, staff and physicians. Provides clinical support to the managers, as assigned.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Reviews telemedicine data and responds according to established protocol. Provides readings to physician as requested. Assures a printout of the data and response actions is filed on the permanent medical record.
  • Responds to incoming calls from patients, caregivers, staff and physicians and facilitates actions to coordinate patient care. Seeks direction from Clinical Supervisors appropriately. Documents calls, care coordination, and physician orders promptly in accordance with agency requirements.  Enters faxed orders. Facilitates communication with physician practices for field staff (verbal or written means).
  • Performs or assists with activities related to waived testing (i.e. INRatio meters), including facilitation of communication with physician practices for field staff, quality assurance data collection and review activities.
  • Performs new referral intake/processing and managed care authorization processing as requested.
  • Assists with quality audits and data collection as requested under the direction of the Director of Quality or designee.
  • Assists Clinical Supervisors, as directed, to facilitate improved performance and efficiency of the clinical staff.
  • Assists with staff immunization activities, as requested.
  • Promotes good public relations with all internal and external customers.
  • Follows all agency policies and procedures

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • LPN – minimum 1 year acute care experience required – cardiac preferred, home care experience preferred.
  • Current licensure in the Commonwealth of Pennsylvania.
  • Current CPR certification.
  • Ability to multi-task and prioritize diverse assignments and function autonomously and interdependently within a complex system.
  • Basic computer skills (word processing, spreadsheets); ability to use basic office equipment.
  • Effective written and verbal communication skills.

  • Ability to add, subtract, multiply, and divide all units of measure.
  • Ability to perform the four operations with like or common decimal fractions. Ability to perform arithmetic operations involving all American monetary units. Using Algebra, to calculate variables and formulas; monomials and polynomials; ratio and proportion variables; and square roots and radicals.

EQUIPMENT USAGE REQUIREMENTS: MACHINES, EQUIPMENT, TOOLS, SOFTWARE: To perform the essential duties of the position, use of the following machines, equipment, tools, and software is customarily required:

PHYSICAL REQUIREMENTS: Degree of physical exertion is light, exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to move objects.

BODY POSITION AND MOVEMENT: The following positions or movements are typical of the job:
