This position will support the Southern Region.

The role of the Ambulatory Collaborative RN Care Manager (ACCT RN) remains an integral part of the Ambulatory Collaborative Care Team (ACCT). ACCT provides focus on our at risk patient population. Providing outreach and education using motivational interviewing skills and teach back; the team is able to work with patients to develop health care management goals and strategies; as well as connect patients to much needed services in their community . The Nurse serves as the primary care manager for the top 5% of patients who have a variety of fundamental and high-risk drivers of complexity of care.

The RN Care Manager's duties include receiving and seeking referrals from usual (planned) care teams of patients appropriate for Collaborative Care Management, stratification of risk based on chart review and provider/team input, patient assessment, medication reconciliation, medication management, referral coordination, assist with patient/caregiver/co-learner goal setting and disease education. The RN care manager works closely with the team's Community Health Worker (CHW) and SW Care Manager to help assess, develop and implement, evaluate and assess the individualized plan of care for their patients.

Hours:  Full-time, day-shift (Mon-Fri) No weekend/holiday hours

Travel may be required to physician practices within the Southern Region.  Travel may also be required for patient home visits.


  • RN:  Current licensure as a Registered Nurse, issued by the Pennsylvania Board of Nursing.  Bachelor’s degree in Nursing (BSN) or a related health care degree is required
  • Three (3) years of acute care nursing experience.
  • Excellent verbal and written communication skills.
  • Excellent Customer service skills.
  • Proven informal leadership skills.
  • Ability to work independently, setting priorities to coordinate care plans efficiently.
  • Ability to work effectively in a fast-paced team environment.
  • Highly organized and detail-oriented with the ability to perform multiple tasks simultaneously.
  • Effective behavioral and educational strategies, including, but not limited to: Motivational interviewing, teach-back method and self-management support.
  • Knowledge of Utilization review or managed care is preferred.
  • Licensure with Commission for Case Managers (CCM), or expected to obtain within two (2) years of employment is preferred.
